Our 2011 National Quality Award winning community in Northern Columbia County, Ohio, just outside of Salem, thrives in a quality culture which is emulated through our staff members. We boast a diverse selection of activities and events which appeal to our residents’ interests and hobbies. Our Community Features
Blossom Nursing and Rehabilitation Center is an expansive single-floor, 100-bed community consisting of 42 semi-private and 16 private resident rooms distributed among four units. Our community is a modern design and features 5 dining rooms, 4 lounges, a chapel, conference room and a large therapy room.
Our Community Services
Blossom Nursing and Rehabilitation Center is a dually certified community. Its professional nursing staff is proud of the exemplary services they deliver including intravenous therapy, wound care, oxygen therapy, and hospice services. The in-house physical, speech, occupational therapists treat both short-term intensive and long-term extensive care residents. Our restorative care compliments the therapies by affirming maximum physical improvement.

We understand the emotional process of placement and strive to help with the adjustment for both the resident and his or her family.
