Required Documentation

When planning an admission to one of our centers, whether you’re being discharged from a hospital or coming directly from your own home, we will need the following to provide excellent care:

Medical History and Physical 

The admitting physician must complete these documents, and they must be current.

Admission or Transfer Orders

These must be signed by your own physician and must include current medication (prescription) orders and any other special orders for care.

Agreement of a Physician to Visit You

Your physician must agree to visit you, in our center, as often as required by regulation or by your condition. If your physician cannot do this, our team will arrange physician coverage for you.

Helpful Documentation

When you transfer to our center, you should also bring any of the following that apply to you:

  • Your Medicare card
  • Your insurance card
  • Your Social Security number
